Download the admission form from the official website www.myholyfaith.org only.

The admission form can be downloaded after paying ₹300 + (transaction charge).

Please enter your valid name, phone number, and email address and click on 'Download Form'. It will redirect you to the payment gateway.

The admission form will be sent to the registered email id provided during the payment process.

If you do not receive the form on your email even after the payment is successful or if you mistakenly deleted or lost the soft copy, kindly provide the payment ID which you have received from Instamojo payment gateway when you made the transaction.
